National Kidney Disease Education Program (NKDEP)

Description of Collaborative Activity:

The National Kidney Disease Education Program (NKDEP) is an initiative of the NIH National Institute of Diabetes and Digestive and Kidney Diseases. NKDEP aims to improve early detection of chronic kidney disease (CKD), facilitate identification of patients at greatest risk for progression to kidney failure, promote evidence-based interventions to slow progression of CKD, and support the coordination of Federal responses to CKD. The NKDEP Health Information Technology (HIT) Working Group is collaborating with the Office of the National Coordinator for Health Information Technology on a CKD Care Plan, the CKD-related elements that should be included in a comprehensive electronic care plan template. The template elements will enable patients and clinicians to access, create, record, change, and receive key patient information and goals across multiple conditions. The HIT Working Group also held a workshop on "Using Health IT to Identify and Manage CKD Populations: Efforts of the NKDEP Health IT Working Groups" in 2017.
