To develop and test: 1) a clinician-facing SMART on FHIR e-care plan application to enable clinicians to view and share comprehensive e-care plan data for people with multiple chronic conditions and 2) an HL7 Implementation Guide (IG) to specify data standards and help healthcare institutions implement the e-care plan. |
