Description of Collaborative Activity: |
The HHS SDoH Workgroup has developed a strategic vision to make health outcomes more equitable by improving the delivery of, and integration between healthcare and social services with a particular focus on individuals and populations in government health and human service programs at high risk for adverse outcomes. The SDoH workgroup identifies how HHS can address SDoH and work with other federal, state, local, tribal, and private sector partners to both ascertain and achieve shared objectives. |
