Convened by the White House Domestic Policy Council and the Office of Science and Technology Policy - the SDOH-IPC purpose is to support a Federal framework for a Coordinated Care Delivery Model to Address SDOH Develop a White House Action Plan sharing guidance and potential policy levers to address Social Determinants of Health through data accessibility, integration and blending and braiding of funds. |
